- Understand the information and care needs of people concerned about or affected by genetic conditions, identified through various NHS and Social Care departments, voluntary services and self-referral.
- Work with professionals to address the information and support needs of individuals/couples/family members. You will be working, in a supporting role, with the professionals responsible for the care of people concerned about or affected by genetic disorders. This may involve clinic attendance and/or home visits.
- Support people affected by genetic conditions to resolve social issues, for example housing, benefits, and welfare, that affect their quality of life, and act as barriers to accessing genetic services. Your role will involve signposting people to relevant services.
- Manage, monitor, and maintain records of people using genetic and support services in a range of sectors. You will be supported in this work by professionals in the Genetics and Good Preconception Care Network.
- Deliver community-based information sessions to improve public understanding of genetics and genetic services, with the aid of existing resources. This will involve working with a variety of organisations, for example, educational establishments, faith organisations and community centres.
- Promote and respond to requests for existing genetic training sessions for health and social care professionals and other stakeholders.
- Organisation of advisory group meetings
- Representation on relevant stakeholder committees
- Oversee the development of the GiC webpage
- Facilitate the development of future resources
- Facilitate the development of genetics pathways
- The post will be supported by an advisory committee and administrative staff.
- Ensure safeguarding practices are known, understood, and effectively implemented.
- Provide person centred care to women and their families, allowing choice and autonomy. Offer support, information, and encouragement to ensure they have full access to care, personalised to their needs/choices.
- Recognise the unique characteristics of each family and implement measures to reduce barriers to good healthcare, considering the socio-economic circumstances of the family including linguistic, cultural, and religious factors.
- Understand the demographics of the community areas within which you work. Recognise that social, cultural/religious influences, individual circumstances, capabilities, behaviours, and lifestyle choices impact on health outcomes.
- Work as a member of the Wellbeing Team and with other professionals enabling care for women, who may have a barrier to accessing the services they need. This will involve being able to work across services in hospital, within the community and home settings.
- Work in partnership to bridge the gap between services and communities.
- Use social networks and patients life experiences to address barriers to engagement and improve connections between services and disadvantaged communities.
- Actively participate in service improvements towards being an outstanding service provider, learning from feedback, enabling, and involving service users to contribute to service design and care choices.
- Assist in making informed choices to enable personalised discussion, sensitively communicating complex, and/or potentially challenging information to women, if appropriate, to facilitate access to services.
- Act sensitively, compassionately, and respectfully at all times.
- Deliver the service in a way that reduces service access, barriers, understanding and responding to the needs of marginalised groups.
- Apply the principles of continuity of carer when making judgements related to supporting women with complex needs and when referring or transferring care back or on to registered practitioners or specialist teams.
- Work with the service towards implementing national standards and improvements in care.
- Acknowledge your personal and professional limitations, always liaising with other relevant health professionals.
- Promote the importance of making every contact count through actively encouraging women and their families to talk about their health and wellbeing at every opportunity.

Job summary
Genetics in Communities Service - 25 hours per week
The Genetics in Communities (GiC) Service is a new support service in Bradford District and Craven and is an integral part of a range of health and social care services. The overall aim of the GiC Service is to ensure that people concerned about or affected by genetic disorders, are sufficiently informed and confident about using available services to improve their health outcomes.
You will be part of a vibrant, dedicated and enthusiastic group of people with a commitment to a family centred and holistic approach to healthcare.
Main duties of the job
Genetics in Communities (GiC) Service Development Coordinator role
Due to the population makeup of the District, initially, you will work with families of Pakistani origin. This role therefore, needs you to communicate confidently in Punjabi/Urdu with women and their family members, whose first language may not be English.
No prior knowledge of genetics is required as full training will be given.
