We are looking for a GP Partner to join our friendly team. We provide excellent primary care and innovative services to a diverse city centre patient population of around 9,000 patients.
Main duties of the job
In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion. Making professional, autonomous decisions in relation to presenting problems, whether self referred or referred from other health care workers within the organisation. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. - Screening patients for disease risk factors and early signs of illness. - In consultation with patients and in line with current Practice disease management protocols, developing care plans for health. - Providing counselling and health education. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate. Recording clear and contemporaneous consultation notes to agreed standards. Collecting data for audit purposes and complying with QOF requirements. Delivering enhanced services. Compiling and issuing computer-generated acute and repeat prescriptions. Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate - In general the post-holder will be expected to undertake all the normal duties and responsibilities
About us
Our practice is a supportive place to work, and we value our dedicated team which currently consists of four GP Partners, two Salaried GPs, a Physicians Associate, Clinical Pharmacist, Physio, Practice Nurses, Health Care Assistant plus our management and admin team. We are active members of our Primary Care network. We work with people experiencing homelessness and there are opportunities to develop skills working to tackle health inequalities.
We are looking for four to six clinical sessions (though have flexibility for the right applicant). We are an active training practice with GP registrars and have a strong history of delivering post-graduate medical education. There are opportunities to develop teaching skills with supervision from experienced educators - we have medical (3B, student selected component) students attached to the practice.
We are a research active practice and interested applicants would have opportunities to develop research skills. We have strong links with the Deep End network of practices, serving the most socioeconomically deprived areas of Yorkshire and the Humber and who are passionate about tackling health inequity. This gives opportunities for further professional development.
We would support early career GPs to access training opportunities including Trailblazer or follow other interests you may bring to the post.
