- Mandatory 4-day Health Coaching Training accredited by The Personalised Care Institute, as set out in the NHS England Workforce Development Framework.
- Knowledge of the personalised care approach and Primary Care Networks.
- Understanding of the determinants of health to include social, economic, and environmental factors.
- Knowledge of public health issues and health inequalities in Herefordshire.
- Understanding of and commitment to, equality, diversity, and inclusion.
- Knowledge of community development approaches and local outreach programmes of work e.g., Talk Community.
- Knowledge of IT systems to include the ability to use word processing skills, emails and the internet.
- Knowledge of the needs of patients with long term conditions.
- Required to travel to work from practices and other locations as required in order to carry out work across the N&W PCN.
- Experience in health promotion and, or health improvement.
- Experience of partnership/collaborative working and of building relationships across a variety of organisations including the voluntary sector.
- Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (can include unpaid work).
- Experience of supporting people, their families, and carers to change health behaviours and improve wellbeing (can include unpaid work).
- Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity.
- Experience of data collection and using tools to measure the impact of services e.g., Patient Activation Measure.
- Experience using clinical systems or similar e.g., EMIS and case management software.
- Excellent communication and interpersonal skills.
- Ability to build rapport and establish one-to-one relationships whilst maintaining appropriate professional boundaries.
- Ability to effectively manage a variable workload.
- Willingness to participate in shaping the future of the provision by working with colleagues across the network and wider community.
- Confidence to engage with local partners to co-produce on out-reach projects.
- Desire to engage with local communities to build resilience and develop assets already in existence.
- Ability to demonstrate emotional resilience and remain composed under pressure.

Working in partnership with GP practices and the wider multidisciplinary team?
We have an opportunity for a Health and Wellbeing Coach to join our PCN Wellbeing team in the North & West Primary Care Network.
You will work closely with GP teams, Care Coordinators, social prescribers, allied health professionals, and community partners. Using personalised care and behaviour change techniques, you'll support people in managing long-term conditions like hypertension, diabetes, and weight management. You'll help individuals build the skills, knowledge, and confidence to take control of their health. A key part of the role is tackling health inequalities and connecting people to local services and community support.
* This is a part time position working 18.75 hours per week*
Main duties of the job
Key responsibilities will include
Coach and motivate patients through 121 sessions to identify their needs and implement change, or through group-based approaches that target specific cohorts.
Provide interventions such as self-management education, enabling positive lifestyle intervention and access to peer support.
Support people to establish and attain goals based on what is important to them for their health and wellbeing.
To work with people with lower activation to understand their level of knowledge, skills, and confidence when engaging with their health and wellbeing.
Utilise existing IT and MDT channels to screen patients to identify those that would benefit from coaching.
Work with Social Prescribers, Public Health community teams and wider partners to co-deliver community-based projects

Take a non-judgemental approach, based on strong communication and negotiation skills that supports personal choice and ensure patients understand the accountability of their own decisions.
Main Responsibilities:
Coach and motivate patients individually through 121 sessions to identify their needs and implement change, or through a group-based approach delivered with colleagues and wider partners that target specific cohorts identified via practice or public health data.
Provide support to individuals, their families and carers to ensure they are active participants in their own healthcare; empowering them to take more control in managing their own health and wellbeing, to live independently, and improve their health outcomes through:
Support colleagues within General Practice and wider to improve their skills and understanding of personalised care, shared decision making and behavioural approaches and ensuring consistency in the follow up of peoples goals where an MDT is involved.
To work with people with lower activation to understand their level of knowledge, skills, and confidence (their Activation level) when engaging with their health and wellbeing.
Utilise existing IT and MDT channels to screen patients, with an aim to identify those that would benefit from Health and Wellbeing Coaching.
