- Be willing to attend training with a non-clinical Supported self management health coaching skills programme (minimum 4 days) by a Personalised Care Institute (PCI) accredited trainer or organisation prior to taking referrals.
- Coaching/counselling qualification/ experience or other relevant qualification/experience involving reflective listening skills relevant training and experience in non-clinical Supported Self-Management (SSM) Health Coaching through a PCI-accredited organisation
- Understanding how to apply health coaching in group settings
- Able to work within a biopsychosocial model, using a range of tools and techniques to enable and support people, such as agenda setting, goal setting, problem solving
- Demonstrable skills in supporting behaviour change
- A good understanding of the evidence base and development of self-management in the UK
- Understanding of the importance and process of helping people with long-term conditions to develop their knowledge, skills and confidence in managing their health and the range of models and tools available.
- Flexibility to work outside of core office hours
- Satisfactory Disclosure Barring Service (DBS) check
- Evidence of continuing professional development
- Access to own transport and ability to travel across the locality on a regular basis

Job summary
Pontefract and Knottingley PCN is recruiting an experienced, enthusiastic and forward thinking Health and Wellbeing Coach to join our established team to cover a population of over 49,000 across 4 practices in the area.
Health and Wellbeing Coaches work as a key part of the primary care network (PCN) multi-disciplinary team. Health and Wellbeing Coaches support people to take pro-active steps to improve the way they manage their health conditions, based on what matters to them. They consider the whole person in addressing existing issues and encouraging the proactive prevention of new illnesses, promoting lifestyle medicine approaches. Health coaching revolves around using coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers.
Main duties of the job
Health and Wellbeing Coaches are an integral part of the general practice team working in each practice across the PCN. The post holders will work in partnership with their clinical and non-clinical colleagues, management support and the wider PCN to ensure the role delivers the best possible outcomes for our patients. Health and Wellbeing Coaches support people develop their knowledge, skills and confidence in managing their health and care, to improve their health outcomes and quality of life. Health coaches do this by using coaching techniques and motivating patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives.
Health and Wellbeing Coaches may work with patients over the phone, by video conference, or face-to-face. The role however is varied, and may include supporting self-management education, peer support, caseload management and facilitating group consultations. Identify opportunities and develop and deliver project recommendations for services and actions needed for active lifestyles that contribute to health outcomes, community safety and independent living in the Primary Care Network.

Health and Wellbeing Coaches will have access to ongoing supervision, skills development, and support so they are able to further build their skills and experience within the role.
There may be a need to work remotely depending on the requirements of the role.
You would be joining the existing ARRS team made up of Health and Wellbeing Coaches, Care Coordinators, Pharmacists, Mental Health Practitioner, Occupational Therapist and a social prescriber (currently Live Well Wakefield).
Please note that the Health and Wellbeing Coach role is a non-clinical role.
