      Job summary

      Purpose of the role

      To have a good understanding of the Health Coach role; to hold an active caseload and provide support as a Team Lead to be able to supervise and mentor the Health Coaches. Additionally, to help with the development of the Health Coach Team to ensure that the service is robust and sustainable

      Health and Wellbeing Coaches support people to take pro-active steps to improve the way they manage their physical and mental health conditions, based on what matters to them. They support people to develop their knowledge, skills, confidence in managing their health and care, to improve their health outcomes and quality of life and support them making changes in health related behaviours.

      Health coaches do this by coaching and motivating people through multiple sessions to identify their needs and to set goals for themselves, and through providing access to interventions such as self-management education and peer support.

      Health and Wellbeing Coaches manage and prioritise a caseload, according to the needs, priorities and support required by individuals in the caseload. They identify when a persons needs are beyond the scope of the health and wellbeing coach role and refer them back to other health professionals or organisations. Health and Wellbeing Coaches may work with people by phone, by video conference, or face-to-face.

      Main duties of the job

      The successful candidate will be kind, reflective and self-aware and will enjoy working with a wide range of people. They will have good communication and negotiation skills and a firm belief that people have untapped resources within them, that can be unleashed by providing high quality, non-judgemental support. They will be proactive with a flexible attitude, keen to work as part of a team and committed to developing themselves in this skilled role and supporting colleagues to do the same.

      Health and Wellbeing Coaches are a key part of the Primary Care Network (PCN) multidisciplinary team. They often work alongside Coordinators and wider PCN MDT and practices to provide an all-encompassing approach to personalised care and promote and embed the personalised care approach across Primary Care Network(s).

      Health and Wellbeing Coaches will have access to ongoing supervision, skills development, and support so they are able to further build their skills and experience within the role.

      Please note that the Health and Wellbeing Coach Team Leader role is a hybrid role and the individual will be expected to maintain an active case load, alongside providing support as a Team Leader.

      The post holder will work alongside the PCN Manager, the Clinical Director and the wider team to achieve success in line with the PCN Network Contract DES and the PCN priorities. Providing support as a Health Coach, along with providing line management as a Team Lead to the PCNs Health Coaches and leading PCN projects.

      As well as delivering the services set out in the PCN Enhanced Service specification as well as expanding proactive care in our community.

      The PCN Team consists of Complex Care GP's, Clinical Pharmacists and Pharmacy Technicians, Occupational Therapists, Health Coaches, Care Coordinators, a Services Development Lead and a Digital Lead.

      The 5 PCN practices are, Axbridge and Wedmore Medical Practice, Brent Area Medical Centre, Burnham Medical Centre, Cheddar Medical Centre, Highbridge Medical Centre.

      We have a PCN Hub, based at the Burnham Health Campus, Brent Road, Burnham on Sea.

      Team Leader Key Responsibilities

      To work with and support the PCNs Clinical Director, PCN Manager, PCN members and their Teams to ensure effective delivery of the PCN DES and IIF requirements

      Responsible for assisting with day-to-day operational delivery

      Support equity of service provision across the Networks 5 core practices

      Ensuring objectives of team is being delivered across all 5 Practices, and when staff absences occur ensure that activities are maintained and delivered.

      Trouble shooting day to day and responding to any queries from Health Coaches and wider team as required.

      Develop and maintain effective communication both with the ARRS staff and with all partner practices.

      To provide support with project implementation and communications for new services.

      Support the PCN to develop/ maintain a patient voice forum, patient facing comms and social media presence for the PCN

      Organise meetings or events related to PCN activity.

      Attend and present at PCN Executive Board and Clinical Leads Meetings

      Where required, take a lead role in project delivery. This could range from individual PCN projects to supporting clinicians in the development of new care pathways.

      Help develop joint working and relationships across networks and its stakeholders.

      Communicate information and issues, including briefings and reports.

      Maintain excellent relationships across all core member Practices.

      To produce, review and circulate documents required for meetings.

      Minute meetings. Collating action points and circulating.

      Supporting PCN members with all other reasonable administration requests.

      Supporting and collating evaluation of PCN services to drive further improvements.

      Help develop policies and procedures that are accurate.

      Support the financial delivery of PCN activities.

      Develop and maintain databases required for the role.

      Work with the PCN member practices to identify and address access barriers for those who struggle to access healthcare.

      Working with other PCN Health Coaches and Care Coordinators, raise awareness of health promotion, screening, NHS Health Checks and LD Health checks with patients.

      Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role.

      HR Duties

      To undertake day-to-day line management of all Health Coaches across the PCN.

      Support the retention of staff and ensure HR needs are met for all Health Coaches.

      To provide line management to other Health Coaches; including management of absence line, managing and arranging cover for staff as required.

      Support the recruitment processes for new PCN staff, from advertising to appointment, as required.

      Arrange and oversee staff induction and training, ensuring staff have completed all training

      Conduct appraisals and support performance management of designated PCN staff as agreed.

      Deliver a high standard of patient care, leading Health Coaches within the network, to work collaboratively with the PCN members to meet the needs of patients.

      Personal and Professional Development

      Keep abreast of national and local guidance on PCNs.

      Take responsibility for own development, learning and performance.

      Participate in an annual individual performance review, and

      take responsibility for maintaining a record of own personal and professional development.

      Health Coach Key Responsibilities

      Provide one-to-one and group health coaching support for people with one or more long-term conditions, based on what is important to them, with the aim of: improving peoples knowledge, skills, confidence in managing their conditions, empowering people to manage their own health and improve their health outcomes and support them in making changes in their health-related behaviour

      Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals in the caseload. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals, agencies, when what the person needs is beyond the scope of the health and wellbeing coach role e.g. when there is a mental health need requiring the patient to be referred to an appropriately qualified practitioner.

      Work as part of a multidisciplinary multi-agency team to promote health coaching and to be ambassadors for Personalised Care and Supported Self-Management, modelling the coaching approach in their work.

      Ensure that GPs, practice nurses, practice pharmacists and other members of the primary care team understand the Health and Wellbeing Coach role, how to refer to them, and which patients may particularly benefit from health coaching.

      Support local health, social care and voluntary sector professionals to make appropriate referrals to the service. Promote and raise awareness of the health coaching service particularly to groups and communities that experience barriers to access.

      Attend and contribute to team, practice, and PCN meetings and events as required by the service.

      Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role.

      Participate in regular health coaching supervision and continual learning. This may include, but is not limited to, any or all the following:

      o Regular contact with service supervisor

      o Refresher training sessions

      o Buddying with peers

      o Peer support sessions

      o 1:1 support from a practitioner with more health coaching experience

      o Action Learning Sets

      o e-learning to revisit or deepen training

      o On-going improvements to systems and processes
