      Job summary

      Were recruiting an experienced, enthusiastic and forward-thinking Health and Wellbeing Coach to join our supportive, growing multi-disciplinary team in Wakefield. Working across several GP practices, youll support people to take proactive steps in managing their physical and mental health, focusing on what matters most to them. Using personalised care and behaviour change coaching techniques, youll help individuals build the knowledge, skills and confidence to better manage long-term conditions like diabetes, hypertension and weight management, while tackling health inequalities and connecting people to community services and support.

      Main duties of the job

      As a Health and Wellbeing Coach, you will work in partnership with GP practice teams, care coordinators, mental health nurses, social prescribers, and the wider MDT to deliver personalised support for patients. You will:

      Use health coaching techniques, lifestyle medicine principles and person-centred care to support people in setting health and wellbeing goals and creating achievable action plans.

      Help individuals develop the knowledge, skills, and confidence to manage long-term conditions such as diabetes, hypertension, and weight management.

      Support patients through one-to-one and group sessions, delivered face-to-face, over the phone, via video calls, and in community venues.

      Work with people experiencing low levels of patient activation to improve their motivation, wellbeing, and independence.

      Facilitate self-management education and wellbeing promotion activities, including group consultations where appropriate.

      Record patient contacts, outcomes, and feedback accurately using relevant clinical systems.

      Contribute to MDT discussions, service development initiatives, and community-based health and wellbeing projects.

      Participate in ongoing supervision, CPD, and peer support opportunities provided by Trinity Health Group PCN and Conexus Healthcare CIC.

      Work flexibly across GP practices, community venues, and occasional remote settings as required.
