      Job summary

      Enfield Unity PCN are looking for an enthusiastic & dedicated health coach to work with our Personalised Care Team, meeting the needs of local communities to reduce health and social inequalities.

      Main duties of the job

      • Support service users to develop healthier eating habits and learn about healthy balanced meals.
      • Promote and support weight loss, or prevent further excess weight gain, among service users living with overweight or obesity
      • Support service users to develop strategies and self-regulatory skills to overcome challenges and self-manage weight maintenance goals
      • Offer smoking cessation support
      • Plan and organise workload, this will include the management of a caseload of clients.
      • Deliver one to one or group stop support sessions, ensuring ease of access for clients from local communities.
      • Provide a high quality, efficient service throughout a diverse range of community settings.
      • Monitor outcomes of treatment programmes for their effectiveness.
      • The advisor may potentially be required to work with complex health needs in relation to behaviour change.
      • In partnership with the service coordinator and manager develop new service initiatives, based on periodic impact needs assessments.
      • Develop and maintain close working relationships with key stakeholders and partners.
      • Ensure a commitment to own personal development by keeping up to date with research and best practice.
      • Attend team meetings and training as required.
      • To actively participate in promotional events.

      About us

      Enfield Unity PCN is comprised of 17 GP Practice sites across Enfield.

      Medicus Health Partners (MHP) is the lead practice for Unity PCN and will be the employing organisation. MHP is a super-partnership made up of 9 of the 17 PCN sites.

      The successful applicants will have access to the training & support offers of this large organisation.

      The successful applicants will be working closely with the PCN Personalised care Team, Clinical Directors and Clinical Leads.

      MAIN DUTIES AND RESPONSIBILITIES

      1. Motivate and empower patients to make behaviour changes around diet and physical activity as well as smoking cessation.

      2. Engage with patients and to provide lifestyle advise and support. This includes delivering basic health checks such as height/weight/BP. A personalised care plan is developed working with the patient, focusing on lifestyle.

      3. Provide a sensitive approach so that individuals accessing the service feel that they are in a safe space and facilitators offer a non-judgemental, empathetic and non-threatening environment.

      4.The service will include a minimum of three full-length consultations with the Healthy Lifestyle advisor (delivered either as a mixture of face-to-face or remote, depending on patient preference and service practicalities)

      5. Plan and organise workload, this will include the management of a caseload of clients.

      6. Deliver one to one telephone and face to face support sessions as well as group sessions on healthy lifestyle including stop smoking support sessions, ensuring ease of access for clients from local communities.

      • Knowledge of behaviour change models.
      • Knowledge of the problems experienced by socially disadvantaged sectors of the community.
      • An understanding of the impact of obesity and smoking on health.
      • Knowledge of dietary approaches represented by the Eatwell Guide.
      • Knowledge of up to date and relevant recommendations on physical activity levels and reducing sedentary behaviour.
      • Working knowledge of the issues related to information governance and the need for confidentiality and how to maintain it.
      • Working knowledge regarding safeguarding processes and policies.
      • Excellent communication skills.
      • Excellent time management and organisation skills.
      • The ability to work independently or as part of a team.
      • The commitment to provide an efficient and effective service.
