      What to Expect (Job Responsibilities):
      - Conduct outreach to individuals needing preventative health services and chronic care management
      - Provide health education and guidance to interrupt ongoing disease transmission
      - Follow standard protocols to obtain medical history and relevant health information
      - Make appropriate referrals to testing, clinical services, and other essential support services
      - Assist with additional duties such as supplies ordering and answering phones during low caseloads

      What is Required (Qualifications):
      - Minimum educational requirement: High school diploma or GED; Associate degree preferred
      - Experience conducting telephone-based or in-person interviews, data collection, and data entry preferred
      - Experience with healthcare, health education, community outreach, or public health services preferred
      - Completion of contact tracing training module within 3 months of starting date
      - Proficiency with computers and mobile devices, including MS Office

      How to Stand Out (Preferred Qualifications):
      - Experience living and working in the Yukon-Kuskokwim Delta preferred
      - Ability to communicate in Yupik or Athabascan is desirable
