      What to Expect (Job Responsibilities):
      - Assist in care coordination of patient/client services by serving as a liaison between the organization and various stakeholders
      - Coordinate and assign daily patient appointments and schedules for clinical field staff
      - Plan client visits based on geographic proximity and staff availability in coordination with Clinical Supervisors
      - Generate and manage scheduling reports to monitor visit completion and identify unassigned visits
      - Communicate essential information to the hospice management team and clinical providers for seamless care coordination

      What is Required (Qualifications):
      - High school diploma or GED equivalent required
      - Minimum of two years of scheduling experience; hospice scheduling experience is highly preferred
      - Strong computer skills, including proficiency in Microsoft Word, Excel, and Teams
      - Critical thinking and problem-solving abilities
      - College degree preferred

      - Experience specifically in hospice scheduling
      - Advanced computer skills or familiarity with healthcare software applications
      - Strong communication skills to facilitate coordination among various teams
      - Background in healthcare or related fields
