What to Expect (Job Responsibilities):
- Develop and implement a housing strategy for MLTSS/SNP/Medicaid members
- Identify and manage resources for housing options
- Educate relevant staff on housing options and supportive services
- Assist members in locating and transitioning to appropriate housing
- Complete reports to meet state requirements and monitor quality
What is Required (Qualifications):
- High School Diploma or GED required
- Minimum of three (3) years full-time experience assisting vulnerable populations with housing
- At least two (2) years of work experience in a public agency or social service field
- Familiarity with public and private housing resources, including HUD and state housing programs
- Strong interpersonal, verbal, and written communication skills
How to Stand Out (Preferred Qualifications):
- Experience in processing prior authorizations and knowledge of Medicare/Medicaid
- Familiarity with care management systems and state-managed entities like HMIS
- Proven problem-solving and conflict resolution abilities
- Strong organizational skills and ability to work independently
