      What to Expect (Job Responsibilities):
      - Identify and connect enrollees to community-based resources for housing, transportation, food insecurity, education, and employment needs
      - Collaborate with Care Managers and Community Health Workers to address social determinants of health (SDOH)
      - Facilitate referrals to community services and programs for enrollees
      - Conduct non-clinical surveys and assist with health screenings as needed
      - Attend community events to educate enrollees about care management services

      What is Required (Qualifications):
      - Must reside in the State of Oklahoma and be able to travel occasionally for meetings and training
      - Minimum of 2 years of experience working with community housing resources or social service agencies
      - Intermediate to advanced knowledge of Microsoft Office Suite
      - Exceptional communication and interpersonal skills to build rapport with customers and stakeholders
      - Ability to multitask and thrive in a fast-paced environment

      How to Stand Out (Preferred Qualifications):
      - Associate degree
      - Related clinical licensure or certification
      - Experience with behavioral change, health promotion, coaching, and wellness
      - Bilingual in Spanish
