What to Expect (Job Responsibilities):
- Perform functions of administrative case management and advocate for individuals with developmental disabilities
- Educate individuals and guardians about service options and ensure service linkage
- Assess Medicaid status and support Medicaid enrollment
- Complete required agency and Department of Human Services forms while maintaining confidentiality
- Manage a caseload of individuals residing in a State Operated Developmental Center (SODC) seeking waiver services or skilled nursing care
What is Required (Qualifications):
- Bachelor's Degree in Human Services or a related field
- Minimum one year of personal or professional experience working with individuals with developmental/intellectual disabilities
- One year of experience working as an Individual Service Coordinator (ISC) in an ISC Program
- QIDP certification in good standing with current caseload
- Valid Driver's License, Insurance, and reliable transportation
How to Stand Out (Preferred Qualifications):
- Two years of experience working as an ISC in an ISC Program
