What to Expect (Job Responsibilities):
- Conduct in-home health evaluations including medication and medical history reviews, physical evaluations, and diagnostic tests as ordered
- Build trusted relationships with patients to improve their health outcomes
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services
- Perform virtual visits as needed to accommodate patient care
- Travel locally and within licensed states to meet patient needs
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification is required
- Ability to cover primary locations, including Bronx and surrounding areas
- Flexibility to perform duties in diverse home environments
- Commitment to providing compassionate care in patients' homes
How to Stand Out (Preferred Qualifications):
- Multi-state licenses preferred for broader coverage
- Experience in in-home health evaluations or similar settings
- Familiarity with using technology for clinical workflows
- Strong interpersonal skills to foster patient engagement
- Previous experience in a healthcare setting focused on patient care
