What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews
- Build trusted relationships with patients to facilitate better health outcomes
- Provide diagnostic and preventive services as indicated
- Perform virtual visits as needed to accommodate patient needs
- Travel locally and within licensed states for patient evaluations
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Ability to cover primary locations: Chautauqua/Buffalo, NY
- Flexibility to travel locally for in-home evaluations
- Commitment to improving patient health outcomes
How to Stand Out (Preferred Qualifications):
- Multi-state licenses (preferred)
- Experience in conducting health evaluations in an in-home setting
- Strong interpersonal skills to engage effectively with patients
- Familiarity with diagnostic and preventive services
- Previous experience as a Nurse Practitioner or Physician Assistant
