What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews, and physical evaluations
- Engage with health plan members to build trust and provide personalized care
- Utilize technology such as an iPad for clinical workflows during evaluations
- Perform diagnostic and preventive services as indicated
- Travel locally and within licensed states as required
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Ability to cover primary locations, including Vancouver and surrounding areas
- Experience in providing in-home health evaluations or similar services
- Commitment to improving health outcomes and patient care
How to Stand Out (Preferred Qualifications):
- Multi-state licenses (preferred)
- Previous experience as a Nurse Practitioner or Physician Assistant in a home health setting
- Skills in conducting virtual health evaluations
- Strong interpersonal skills and ability to connect with diverse populations
- Experience in utilizing technology for clinical workflows
