What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews, physical evaluations, and diagnostic tests as needed
- Build trusted relationships with patients to enhance their health and well-being
- Collaborate with a national network of clinicians to provide comprehensive care
- Flexibly travel locally within licensed states and conduct virtual visits as necessary
- Provide all required diagnostic and preventive services products during evaluations
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification is required
- Ability to cover primary locations: Herkimer/Utica, NY, and surrounding areas
- Flexibility to travel for in-home evaluations
- Experience in conducting patient evaluations in a home setting
How to Stand Out (Preferred Qualifications):
- Multi-state licenses are preferred
- Prior experience as a Nurse Practitioner or Physician Assistant in a healthcare setting
- Strong communication skills and ability to engage with diverse patient populations
- Experience with diagnostic and preventive services
- Familiarity with electronic health record systems
