What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews, physical evaluations, and additional diagnostic tests as ordered
- Build trusted relationships with patients to enhance their health and well-being
- Utilize technology, including an iPad, to facilitate clinical workflows during evaluations
- Travel locally within licensed states as needed and conduct virtual visits when required
- Provide diagnostic and preventive services, such as spirometry and bone density screenings
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Willingness to travel locally and within licensed states
- Ability to perform duties in diverse home environments
- Minimum of 29.5 hours per week availability

- Multi-state licenses
- Experience working in home health or similar settings
- Strong interpersonal skills to engage effectively with patients
- Ability to manage time efficiently and prioritize patient care
- Familiarity with technology used in clinical evaluations
