What to Expect (Job Responsibilities):
- Conduct in-home health evaluations and provide personalized care to patients
- Engage with patients to build trusted relationships and address their health questions
- Perform medication and medical history reviews, and physical evaluations
- Utilize technology for clinical workflows during evaluations
- Travel locally and conduct virtual visits as necessary
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification in relevant field
- Ability to cover specified primary locations, including Penobscot, ME
- Flexibility to travel locally and within licensed states as needed
- Minimum of 29.5 hours per week availability

- Multi-state licenses
- Experience in conducting in-home health evaluations
- Strong communication and interpersonal skills
- Passion for community health and patient engagement
- Familiarity with diagnostic and preventive services
