What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews, and physical assessments
- Engage with patients to answer health-related questions and provide personalized care
- Utilize an iPad and clinical workflow for evaluations without prescribing medications or altering treatment regimens
- Perform virtual visits as necessary and provide various diagnostic and preventive services
- Travel locally within licensed states to meet patients in their homes
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification is required
- Flexibility to cover primary locations, including Rochester and surrounding areas
- Ability to perform duties in diverse home environments with potential allergens
- Commitment to providing compassionate care in patients' homes
How to Stand Out (Preferred Qualifications):
- Multi-state licenses are preferred
- Experience in conducting in-home health evaluations
- Familiarity with using technology for health assessments
- Strong communication skills to effectively engage with patients and their families
- Previous experience as a Nurse Practitioner or Physician Assistant in a healthcare setting
