What to Expect (Job Responsibilities):
- Conduct in-home health evaluations for patients, including medication and medical history reviews
- Engage with patients to answer health-related questions and provide personalized care
- Utilize digital tools, such as an iPad, to facilitate clinical workflows during evaluations
- Travel locally and within licensed states as needed to provide care to patients
- Conduct virtual visits when required to enhance patient access to care
What is Required (Qualifications):
- Active, unrestricted license(s) in the coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to cover primary locations, specifically Marshall, MN
- Flexibility to adapt to diverse home environments while performing evaluations
- Commitment to providing compassionate care and building trusted relationships with patients

- Previous experience working in home health or community-based settings
