What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication reviews, physical evaluations, and diagnostic tests as needed
- Engage with patients to build trusted relationships and improve health outcomes
- Utilize technology, including an iPad and clinical workflow tools, for evaluations
- Travel locally and within licensed states to meet patient needs
- Provide all designated services, including diagnostic and preventive care products
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification is required
- Ability to cover primary locations, including Rochester and the surrounding area
- Flexibility to travel locally and conduct virtual visits as necessary
- Minimum of 12 hours per week availability
How to Stand Out (Preferred Qualifications):
- Multi-state licenses are preferred
- Experience conducting in-home patient evaluations
- Strong interpersonal skills to foster patient engagement
- Familiarity with digital health tools and technologies
- Previous experience in a clinical setting with direct patient care
