What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews, physical evaluations, and potential diagnostic tests
- Build trusted relationships with patients to enhance their health outcomes
- Travel locally and within licensed states as required for home visits
- Use an iPad and clinical workflow for efficient evaluations
- Provide all indicated services, including Diagnostic & Preventive Services products
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Flexibility to perform duties in diverse home environments
- Ability to engage with patients compassionately and effectively
- Willingness to travel locally and conduct virtual visits
How to Stand Out (Preferred Qualifications):
- Multi-state licenses are preferred
- Experience in providing in-home healthcare services
- Strong interpersonal and communication skills
- Familiarity with using technology for clinical workflows
- Commitment to improving community health outcomes
