What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews
- Build trusted relationships with health plan members to improve health outcomes
- Utilize technology, such as an iPad, to streamline the evaluation process
- Travel locally within licensed states to meet with members in their homes
- Provide various diagnostic and preventive services as indicated
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification is required
- Flexibility to travel locally and conduct virtual visits as needed
- Ability to perform duties comfortably in diverse home environments
- Commitment to engaging with health plan members and addressing their unique needs
How to Stand Out (Preferred Qualifications):
- Multi-state licenses are preferred
- Experience in providing in-home health evaluations
- Strong interpersonal skills for effective communication with patients
- Knowledge of diagnostic and preventive services in a clinical setting
- Familiarity with technology used in clinical workflows
