What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews, physical evaluations, and additional diagnostic tests as ordered
- Build trusted relationships with health plan members to enhance their health outcomes
- Provide virtual visits as needed to accommodate patient care
- Travel locally and within licensed states to conduct evaluations
- Deliver diagnostic and preventive services as indicated, such as spirometry and bone density screenings
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner (NP) or Physician Assistant (PA)
- Ability to comfortably perform duties in diverse home environments
- Flexibility to travel locally as needed for patient visits
- Willingness to engage with patients and address their health questions and concerns

- Multi-state licenses for practice
- Previous experience conducting in-home health evaluations
- Strong interpersonal skills and ability to connect with patients
- Familiarity with using technology, such as iPads, for clinical workflows
