What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews
- Build trusted relationships with patients to enhance their health outcomes
- Perform virtual visits as required and provide diagnostic and preventive services
- Travel locally within licensed states to meet patients where they are
- Utilize an iPad and clinical workflow for evaluations without prescribing medication or altering treatment regimens
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to comfortably perform duties in diverse home environments
- Flexibility to travel locally and conduct virtual visits as needed
- Commitment to providing compassionate and comprehensive patient care
