What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews and physical assessments
- Build trusted relationships with patients to enhance their health outcomes
- Perform necessary diagnostic tests as ordered by health plans
- Travel locally within licensed states and conduct virtual visits as needed
- Provide all specified services, including diagnostic and preventive care products
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification in relevant field
- Flexibility to travel locally and within licensed states
- Ability to perform duties in diverse home environments
- Experience in providing patient-centered care
How to Stand Out (Preferred Qualifications):
- Multi-state licenses
- Previous experience conducting in-home health evaluations
- Strong communication skills and ability to engage with diverse populations
- Familiarity with electronic health records and clinical workflow tools
- Commitment to patient education and community health improvement
