What to Expect (Job Responsibilities):
- Conduct in-home health evaluations for patients, including medication and medical history reviews
- Engage with patients to answer health questions and provide personalized care
- Utilize technology to streamline the evaluation process and document findings
- Perform additional diagnostic tests as ordered by health plans
- Travel locally to conduct evaluations in various home environments
What is Required (Qualifications):
- Active, unrestricted license(s) in the coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to perform duties in diverse home environments, including those with pets or allergens
- Flexibility to conduct virtual visits as needed
- Commitment to improving health outcomes through patient engagement
How to Stand Out (Preferred Qualifications):
- Multi-state licenses in addition to primary licensure
- Experience in conducting home health evaluations
- Familiarity with diagnostic and preventive services
- Strong communication and interpersonal skills
- Passion for community health and patient-centered care
