What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews
- Build trusted relationships with patients to support their health journey
- Perform diagnostic tests as ordered by health plans, such as diabetic eye exams and spirometry
- Travel locally within licensed states to conduct evaluations
- Conduct virtual visits as needed to accommodate patient preferences
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to work comfortably in diverse home environments
- Flexibility to travel locally and conduct virtual visits
- Strong interpersonal skills to engage effectively with patients

- Previous experience in providing in-home health evaluations
- Familiarity with diagnostic and preventive services products
