What to Expect (Job Responsibilities):
- Complete assessments to determine individual needs and preferences for community living
- Engage and educate members on recovery principles, employment, and education opportunities
- Connect members to services and housing supports, including peer support services and tenancy rights education
- Collaborate with care teams and external stakeholders to maximize member experience
- Maintain accurate caseloads and tracking tools
- Ensure timely documentation of in reach activities
What is Required (Qualifications):
- High school diploma or GED
- Minimum of two years of experience working with individuals with behavioral health needs
- Lived experience in navigating Mental Health, Public Health, Social Service, and/or Justice systems
- Knowledge of Medicaid benefits plans and community resources
- Strong interpersonal and written/verbal communication skills
How to Stand Out (Preferred Qualifications):
- QP status preferred
- Bachelor's degree in human services or related field
- Master's degree in Human Service-Related Field highly preferred
- Completion of training in WRAP, Person-Centered Thinking, WHAM, Trauma Informed Care, and other relevant areas
