What to Expect (Job Responsibilities):
- Complete MDS assessments, including Medicare PPS and COBRA required assessments, as directed by leadership
- Ensure accurate and timely completion of assessments in compliance with CMS regulations and guidelines
- Conduct comprehensive Care Area Assessments (CAAs) and other care planning requirements as directed
- Attend interdisciplinary meetings and communicate assessment status effectively to the facility team
- Perform general activities consistent with industry norms specific to the position
What is Required (Qualifications):
- Proficient with PDPM and case mix reimbursement
- Experience working with Quality Measures and Five Star
- Strong computer skills, with knowledge of PCC and Net Health, and eagerness to learn new EMRs
- Must have the ability to work independently and as part of a team
How to Stand Out (Preferred Qualifications):
- RAC-CT certification
