      Job summary

      The post holder will be involved with the safe and secure handling of medicines assessments (medicines security), analysing prescribing data into the Practice clinical team.

      The post holder will work closely with the Practice doctors to support the delivery of a high standard of medicines management to Railway Medical Group. The post holder will also liaise with a wide range of staff within the practice, patients, local care homes, pharmacies and local acute hospital trusts.

      Main duties of the job

      Reconciliation of medication on hospital discharge letters and clinic letters.

      Updating patient records and liaising with patients and pharmacies where necessary

      Communicate information about medicines such as their common drug dosage and use to patients, care workers, nurses and medical doctors

      Communicate medicines management and other technical information or aim to address identified issues or gaps to a wide range of people including nurses, allied healthcare professionals and doctors.

      • Reduce waste by decreasing inappropriate issue of prescriptions
      • Ensure review and authorisation dates are actioned and updated on a daily basis
      • Work within the practice to promote and co-ordinate the Repeat Dispensing Scheme as appropriate
      • Work with and train practice receptionists dealing with repeat prescribing, making the best use of their skills
      • Assist the prescriptions team on a regular basis, depending on business need
      • Collect data to monitor improvements in line with core medicine management objectives. Undertake clinical audits
      • Encourage joint working and facilitate integration of all staff towards common practice prescribing aims
      • Engage with local community pharmacists to improve communication and working relations
        • Attend relevant courses and training events
        • Keep abreast of all medicine management issues and ensure practice protocols are updated
