      Job summary

      The creation of the NHS Executive from 1 April 2023 is an essential part of making the health system in Wales fit for the future and achieving A Healthier Wales vision of a whole-system approach to providing excellent health and social care across Wales.Working on behalf of the Welsh Government, the Executive's role is to provide strong leadership and strategic direction and enable and support the delivery of national priorities and standards.

      Transition should be a purposeful and planned process provided as a core component of a person-centred developmentally and physically appropriate healthcare. In line with UNCRC, all children and young people should be actively involved in decisions about their health care. Without effective transition processes it is known that the outcomes for children and young people are worse in adulthood. Effective patient directed and centred transition and planning is therefore essential if we are to consider the upstream effect on the health of adults in Wales.

      Main duties of the job

      Reporting to the Child Health Clinical Lead, the post-holder will: -

      Support the National Clinical Lead for Child Health to assess, monitor and provide assurance in relation to transition of children and young people who transition from children's to adult services.

      With the support of the Child Health Network Team, establish a Transition Implementation Network for Wales.

      Co-ordinate and implement the NHS Wales' response to the review of the implementation of the national transition guidance.

      Collaborate with the transition leads in each of the health boards to improve transition arrangements for children and young people.

      The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

      This post is a secondment opportunity/ fixed term until 31st March 2026 due to funding.

      • Master's qualification; or equivalent qualification; or equivalent level of knowledge and skills.
      • Evidence of current relevant professional registration.
      • Degree, or equivalent, or post graduate qualification in relevant clinical speciality.
      • Member of professional body or covered by professional indemnity insurance.

      Desirable

      • Active membership of relevant special interest groups.
      • Attendance at relevant accredited courses/ conferences.
      • Experience of research and audit.
