What to Expect (Job Responsibilities):
- Provide evaluations and treatment using sound clinical judgment for non-life or limb threatening conditions
- Triage patient concerns to the appropriate level of care and manage urgent and non-urgent issues
- Collaborate with interdisciplinary teams to support coordinated, value-based care
- Educate patients, families, and caregivers on health maintenance and chronic disease management
- Participate in quality improvement initiatives and maintain compliance with required trainings and documentation
What is Required (Qualifications):
- Active and unencumbered nurse practitioner licenses in all states where the employer operates
- Current DEA license in all states where the employer is located
- Experience in call center patient care preferred
- Experience in geriatric acute care
- Flexible, self-driven, collaborative, and innovative personality
