What to Expect (Job Responsibilities):
- Conduct in-home assessments on patients, including medication reviews and health screenings
- Provide patient education by aligning care desired with the best care options available
- Identify diagnoses for assessment in care management and active medical management
- Collaborate with the Regional Medical Officer and team members to develop appropriate care plans
- Coordinate care with multiple stakeholders, including primary care physicians, specialists, and ancillary providers
What is Required (Qualifications):
- Master’s degree from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
- Active Nurse Practitioner or Physician Assistant license
- Active RN license and furnishing number (NP only)
- Active Nurse Practitioner Board Certification/Physician Assistant Certification
- Valid Driver License and current automobile insurance
How to Stand Out (Preferred Qualifications):
- One (1) year of prior clinical or home care experience
- Previous experience with electronic medical records (EMR) preferred
- Experience in geriatric patient care preferred
- Strong problem-solving, organizational, and time management skills
- Ability to communicate positively and professionally with others
