      What to Expect (Job Responsibilities):
      - Conduct in-home assessments on health plan members and perform outpatient clinical work
      - Obtain comprehensive patient history, physical exams, medication reviews, and appropriate screenings
      - Engage in end-of-life care discussions with patients and families
      - Collect laboratory specimens in the home setting when appropriate
      - Communicate assessment findings to the primary care provider to address potential gaps in care

      What is Required (Qualifications):
      - Must possess an active California state Nurse Practitioner or Physician Assistant license
      - Master's degree from an accredited NP Program or PA program
      - Ability to communicate positively and effectively with others
      - Strong problem-solving, organizational, and time management skills
      - Valid California driver license and current automobile insurance

      How to Stand Out (Preferred Qualifications):
      - One (1) year of prior clinical or home care experience
      - Previous experience with electronic medical records (EMR)
      - Experience in caring for older adult (geriatric) patients
