What to Expect (Job Responsibilities):
- Conduct in-home assessments on patients, including medication reviews and health screenings
- Provide patient education by aligning desired care with the best possible care options
- Identify diagnoses for assessment in care management and active medical management
- Collaborate with the Regional Medical Officer and other team members to develop appropriate care plans
- Coordinate care with various stakeholders, including primary care providers, specialists, and ancillary providers
What is Required (Qualifications):
- Master's degree from an accredited Nurse Practitioner or Physician Assistant program
- Active Nurse Practitioner or Physician Assistant license
- Active RN license and furnishing number (for Nurse Practitioners)
- Ability to communicate positively, professionally, and effectively with others
- Strong problem-solving, organizational, and time management skills
How to Stand Out (Preferred Qualifications):
- One (1) year of prior clinical or home care experience
- Previous experience with electronic medical records (EMR)
- Experience in geriatric patient care
