      What to Expect (Job Responsibilities):
      - Conduct in-home assessments and outpatient clinical work for health plan members
      - Obtain comprehensive patient history, physical exams, medication reviews, and conduct cognitive/depression/safety screenings
      - Engage in end-of-life care discussions with patients and families
      - Collect laboratory specimens in the home setting as appropriate
      - Communicate assessment findings to primary care providers to address potential gaps in care

      What is Required (Qualifications):
      - Active California state Nurse Practitioner or Physician Assistant license
      - Master's degree from an accredited Nurse Practitioner or Physician Assistant program
      - Ability to communicate positively and effectively with others
      - Effective problem-solving and organizational skills in a fast-paced environment
      - Valid California driver license and current automobile insurance

      How to Stand Out (Preferred Qualifications):
      - One (1) year of prior clinical or home care experience
      - Previous experience with electronic medical records (EMR)
      - Experience in caring for older adult (geriatric) patients
      - Active RN license and furnishing number (for Nurse Practitioners only)
      - NPI number, DEA, and valid BLS certification
