      What to Expect (Job Responsibilities):
      - Conduct hospital-based assessments and develop treatment plans for high-care needs members
      - Perform in-home assessments, including comprehensive history, physical exam, and medication review
      - Collaborate with market MDs to identify and manage critically ill members
      - Provide patient and family education, emphasizing monitoring and follow-up on patient needs
      - Ensure compliance with HIPAA regulations and maintain the security of protected health information

      What is Required (Qualifications):
      - Active North Carolina state license as a Nurse Practitioner or Physician Assistant
      - At least one (1) year of prior clinical experience
      - Active Nurse Practitioner or Physician Assistant Board Certification
      - Valid BLS (Basic Life Support) certification (Must, upon start)
      - Medicare Part D Prescriber certification

      How to Stand Out (Preferred Qualifications):
      - Experience in the care of older adult (geriatric) patients
      - Home care experience
      - Valid ACLS (Advanced Cardiovascular Life Support) certification
      - Previous EPIC experience
      - Excellent administrative, organizational, and verbal communication skills
