What to Expect (Job Responsibilities):
- Identify community-based organizations as potential referral partners
- Develop relationships with referral partners and conduct presentations to educate staff on program services
- Provide face-to-face outreach, orientation, and screening for referred members
- Utilize telephonic and electronic outreach to obtain necessary information for enrollment
- Maintain accurate case records in designated systems
What is Required (Qualifications):
- High School diploma
- 1 year of relevant experience
- Bilingual in English and Spanish is preferred
- Excellent public speaking, interpersonal, and time management skills
How to Stand Out (Preferred Qualifications):
- Experience in outreach, enrollment, or community engagement roles
- Knowledge of health home programs and services
