What to Expect (Job Responsibilities):
- Provide virtual and telephonic support to a roster of patients
- Conduct comprehensive nutrition assessments and develop individualized meal plans
- Deliver diabetes self-management education and support (DSMES) to patients
- Document all interactions and outcomes using the designated portal
- Collaborate with cross-disciplinary teams to prioritize whole-person care
What is Required (Qualifications):
- Registered Dietitian (RD/RDN) credential through the Commission on Dietetic Registration (CDR)
- Licensed in your state of residence
- Certified Diabetes Care and Education Specialist (CDCES) credential from the Certification Board for Diabetes Care and Education
- Minimum of 5 years of experience preferred
- Bachelor's or Master's degree in nutrition, dietetics, or a related field
How to Stand Out (Preferred Qualifications):
- Strong knowledge of nutrition therapy for various health conditions, including diabetes and cardiovascular health
- Excellent communication and behavior change counseling skills
- Experience working with diverse social, cultural, and socioeconomic backgrounds
- Self-starter with a willingness to learn new skills
- Proficiency in Microsoft 365 and EHR systems
