- To be committed to addressing Health Inequalities.
- To champion the work undertaken in the PCNs around Health Inequalities.
- To lead the Health Inequalities work for the Federation & support the delivery of the Health Inequalities Directed Enhanced Service specification including the proactive social prescribing element.
- Directly line manage services which are relevant and focused on supporting addressing health inequalities
- Work with supporting the Transformation Team on projects which have a focus on addressing health inequalities
- To be committed to addressing Health Inequalities.
- To champion the work undertaken in the PCNs around Health Inequalities.
- To lead the Health Inequalities work for the Federation & support the delivery of the Health Inequalities Directed Enhanced Service specification including the proactive social prescribing element.
- Directly line manage services which are relevant and focused on supporting addressing health inequalities
- Work with supporting the Transformation Team on projects which have a focus on addressing health inequalities
- To support the PCNs to identify patients who may struggle accessing the care they need.
- Focusing on patients with mental health and learning difficulties or those groups experiencing health inequalities in access to care.
- Liaising with PCN staff and facilitate sign posting to services provided by voluntary and community groups linking into developing community assets
- To be responsible for ensuring community engagement work takes place ensuring aims and objectives are met.
- To support the PCNs team to engage with wider system strategies ensuring that health inequalities are being addressed.
- National level: to engage and share local approaches to address health inequalities (as well as Membership of the national Health Inequalities Improvement Forum and access to NHS Future Collaborative Platform)
- System level: to represent the general practice neighbourhood and community partners in the ICS inequalities improvement agenda.
- Place level: supporting the integration of primary care, secondary and community services, in collaboration with peer organisational HI Leads, whilst driving the change to help mitigate inequality. Engaging with other place- based partnerships.
- Neighbourhood / Community Level: to build collaborative, sustainable and positive relationships with external partners to develop a shared vision and understanding of the issues and challenges and how to work together to meet the needs of the community ensuring that it is from a population perspective.
- Bring creative and using meaningful ways of supporting our preventative and proactive approach.
- To oversee data sharing between the Health and Wellbeing Team, PCNs, Federation and sources such as Population Health Management and Health Inequalities systems to keep the team up to date with the latest data and knowledge.
- To progress tackling neighbourhood health inequalities service through empowerment and leadership.
- To strategize for resources and more Additional Role Reimbursement Scheme (ARRS) recruitment, to enable the continuing support of the Health Inequalities work.
- To ensure clear communication between each PCN to share experience and learning - further enhancing the approaches for each programme.
- Support internal and external meetings, representing the PCNs to update on the Health Inequalities programme.
- Spread the learning and capture primary care & community engagement
- Work with other system partners on a population perspective.

- A management qualification or equivalent senior experience in health and wellbeing or other relevant area
- A demonstrable interest in population health and health inequalities
- Minimum of relevant 3 years post-qualification experience
- Relevant postgraduate qualification
- Experience of effective leadership
- Experience of successfully initiating and managing health and wellbeing related projects or services
- Experience and understanding of the dynamics of a diverse group of people and able to diffuse potential areas of conflict
- Experience in GP Practice/Primary Care
- Experience and expertise in Safeguarding and working with Vulnerable Adults
- Experience in working in a Health Inequalities role previously
- Knowledge of health inequality issues and understanding of the NHS and other care provision
- Knowledge of and commitment to the principles of equalities and diversity and its practical implementation
- Knowledge and experience of working within the borough of Westminster
- Ability to identify opportunities to develop existing and new services
- Ability to lead and manage a team successfully with a focus on both development and delegation
- Ability to be an excellent ambassador for the Federation, building meaningful relationships with both existing and new contracts

Job summary
Health inequalities are avoidable, unfair and systematic differences in health between different groups of people. Health inequalities are experienced between different groups of people and are often analysed across four main categories:
Socio-economic factors (for example, income);
Geography (for example, region);
Specific characteristics (for example, ethnicity or sexuality) and
Socially excluded groups (people who are asylum seekers or experiencing homeless).
Cultural differences are a big factor relating to the uptake of cancer screening, health checks, the importance of immunisations (flu, Covid-19, baby immunisations etc), and the understanding of accessing and utilising primary care services effectively.
We require a PCN Health Inequalities Lead to work on behalf of the 4 PCNs to pump prime the Boroughs Health Inequalities to take forward the internal work of the federation. The role will liaise with wider partners, develop project work streams and ensure that a full business case is in place for future developments in line with the Integrated Neighbourhood Teams. The role will work closely with the ICB Bi-Borough Health Inequalities lead. The role will require the line management of relevant service managers/operational leads within the Federation and close working with the project and transformation team.

Healthcare Central London Ltd (HCL) is the GP Federation owned by our 33 General Practices covering the Central London (Westminster) area. The organisation supports 4 Primary Care Networks (PCNs). We operate several NHS contracts on behalf of our PCNs including a Community Dermatology Service; Community Cardiology Service and a Secondary Care Referral Service and an out-of-hospitals provision which is sub-contracted to our 33 General Practices.
On behalf of our practices we are the host employers of a large, and rapidly growing team of ARRS (AdditionalRoles Reimbursementscheme) roles including Clinical Pharmacists; Pharmacy Technicians; First Contact Physiotherapists; Dieticians; Paramedics; Social Prescribers; Care Coordinators, Digital & Transformation Leads, Nursing Associates, GPAs and Care Co-ordinators.
