What to Expect (Job Responsibilities):
- Engage with a collaborative team of healthcare professionals, including health coaches and physicians
- Conduct health assessments, including reviewing laboratory results and medical history
- Assess symptoms and treat appropriately, collaborating with the member's primary care provider
- Deliver care using telemedicine and document in the clinical platform
- Provide patient education to promote habits that prevent diseases and maintain good health
What is Required (Qualifications):
- Physician Assistant license, licensed in multiple states, with an active Georgia state license
- 3-5 years of experience as an advanced practice provider
- Clinical experience working with diverse populations, including underserved groups
- Board-certified in family or internal medicine
- Proven experience in remote patient management across different geographic areas
