What to Expect (Job Responsibilities):
- Engage with a collaborative team of healthcare professionals, including health coaches and physicians
- Conduct health assessments and review laboratory results, medical history, and psychosocial history
- Manage a high-risk population, addressing social determinants of health and specific needs
- Provide patient education to promote healthy habits and disease prevention
- Document care and deliver services using telemedicine through the employer's clinical platform
What is Required (Qualifications):
- Physician Assistant license, with active Indiana state license and multiple state licenses preferred
- 3-5 years of experience as an advanced practice provider
- Clinical experience working with diverse populations, including underserved groups
- Proven experience managing patients remotely across various geographic areas
- Board certification in family or internal medicine
