      What to Expect (Job Responsibilities):
      - Provide frontline care to patients experiencing homelessness, behavioral health challenges, and complex chronic conditions
      - Conduct patient assessments, perform physical exams, and prescribe treatment under physician supervision
      - Collaborate daily with a multidisciplinary team to prioritize and coordinate patient care
      - Design and implement care plans that support patients towards stability and improved health outcomes
      - Document all encounters to maintain accurate and up-to-date records for continuity of care

      What is Required (Qualifications):
      - Active, unrestricted California Physician Assistant License by the time of hire
      - Successful completion of the Physician Assistant National Certificate Exam
      - Basic Life Support Certification
      - Ability to travel up to 75% of work time with a valid CA Driver's License and reliable, insured vehicle
      - Strong interpersonal and communication skills to effectively engage with patients and healthcare team members

      How to Stand Out (Preferred Qualifications):
      - Bilingual in English and Spanish strongly preferred
      - 1+ years of experience working with vulnerable populations, particularly those served by CalAIM
