What to Expect (Job Responsibilities):
- Provide frontline care to patients experiencing homelessness, behavioral health challenges, and complex chronic conditions in the field
- Conduct patient assessments, gather histories, perform physical exams, and prescribe treatment under physician supervision
- Design and implement care plans to support patients' health outcomes and stability
- Collaborate with a multidisciplinary team to prioritize and coordinate care
- Document all encounters to maintain accurate, up-to-date records for continuity of care
What is Required (Qualifications):
- Active, unrestricted California Physician Assistant License by the time of hire
- Successful completion of the Physician Assistant National Certificate Exam
- Basic Life Support Certification
- Valid CA Driver's License and ability to travel up to 75% of work time
- Strong interpersonal and communication skills
How to Stand Out (Preferred Qualifications):
- Bilingual in English and Spanish strongly preferred
- 1+ years of experience working with CalAIM populations, including those experiencing homelessness and adults with complex medical needs
