What to Expect (Job Responsibilities):
- Deliver personalized high-quality care to seniors in various settings including homes, assisted living, hospice, and nursing facilities
- Provide dementia-informed, relationship-based care across the continuum of care
- Conduct comprehensive assessments and develop tailored care plans for older adults
- Manage chronic conditions with empathy and skill for patients and their families
- Collaborate with multidisciplinary teams to deliver coordinated, whole-person care
What is Required (Qualifications):
- Active, unencumbered NP or PA license in the state of practice
- Minimum of 1-2 years of clinical experience, preferably in geriatrics or primary care
- Compassionate, patient-centered approach with strong clinical judgment
- Valid driver's license and reliable transportation
How to Stand Out (Preferred Qualifications):
- Experience in dementia-informed care and caregiver support
- Background in palliative or psych care
- Demonstrated ability to build trusted relationships with residents and caregivers
