What to Expect (Job Responsibilities):
- Deliver personalized high-quality care to seniors in various settings, including homes, assisted living, hospice, and nursing facilities
- Conduct comprehensive assessments and develop tailored care plans for older adults
- Manage chronic conditions with empathy and skill, supporting both patients and their families
- Collaborate with cross-functional partners to provide coordinated, whole-person care
- Build trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals
What is Required (Qualifications):
- Active, unencumbered Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of practice
- Minimum of 1-2 years of clinical experience, preferably in geriatrics or primary care
- Compassionate, patient-centered approach and strong clinical judgment
- Valid driver's license and reliable transportation

- Experience in dementia-informed care and caregiver support
- Familiarity with palliative, psych, and neurology services
