      PO5: £48,747 pa inc LW rising in annual increments to £51,783 pa

      Secondment Opportunity/Fixed Term full time until 31 March 2025

      We have a new development opportunity for a Programme Manager to join the Integrated Commissioning Team for adults. The postholder will lead on the delivery of a high-profile programme to mobilise a new Neighbourhood model of care for home care services.

      This is an exciting time as we prepare to implement new Neighbourhood contracts for home care. The Neighbourhood model of care benefits clients through greater integration, continuity of care and use of community assets. The model also allows carers to respond more quickly to an individual’s changing needs and minimise time spent on travel.

      The potholder will lead a small project team to work on the mobilisation of the new Neighbourhood model of care, from contract award to the phased go-live of new Neighbourhood areas.

      We are ideally seeking a candidate with experience of working in health or social care, Public Health, commissioning or a similar field and a strong track record of providing effective programme management to deliver results. Experience of working on complex change programmes within social care would be an advantage but is not essential.
