- Ensuring the implementation of the client's treatment plan(s), from admission through discharge and follow-up
- Coordinates schedules, therapies and other appointments/services
- Responsible for regular contact with families and funders
- Coordination with medical director and external medical providers to ensure appropriate medical care
- Collaborates with the program nurse and clinical director to ensure clinical and medical needs are being met.
- Communicates with the Operations Director to ensure residential care is being met and any personal preferences are identified.
- Report writing, clinical documentation and facilitating funding issues as needed
- Acts as a client advocate

Learning Services, A Collage Rehabilitation Partner is looking for a full-time Rehabilitation Case Manager in Gilroy, CA. Learning Services is a growing organization dedicated to excellence, honesty and ethics in the treatment of our clients and each other. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries.
Responsibilities:The Case Manager is responsible for:
Pay is based off of education and experience: $80,000 - $90,000 annually
Qualifications:Benefits include, but are not limited to:

? A bachelors degree along with case management in the healthcare industry/rehabilitation experience is
required. A masters degree and CRC or CCM designation is preferred
? Must have a valid driver's license and maintain responsible driving behaviors
? Excellent planning, organizational, multitasking, writing and time management skills are required
? The ability to communicate with families, professionals and team members is a must
? Strong clinical skills as shown by the ability to manage crisis situations, assess clients needs and help
formulate, prioritize and monitor treatment plans is necessary
