      What to Expect (Job Responsibilities):
      - Engage, motivate, coach, and educate caregivers via phone and digital technology
      - Assess caregiver and care recipient needs, coaching caregivers to develop and achieve goals
      - Conduct face-to-face semi-annual meetings with clients, caregivers, and case managers
      - Communicate with key referral sources and healthcare providers to promote services
      - Proactively identify and cultivate new prospects to expand the client base

      What is Required (Qualifications):
      - Associate's Degree or 2+ years of experience in community health, long-term care, or chronic diseases of adults
      - 2+ years of experience in case management, specifically working with elderly and disabled individuals
      - Exceptional active listening and communication skills with strong empathy
      - Proficiency with Microsoft Office and ability to multitask in a multi-system environment
      - Sales experience with a focus on building relationships and pipelines

      How to Stand Out (Preferred Qualifications):
      - Experience in a fast-paced, start-up environment
      - Comfort using and teaching others to use mobile apps
      - Strong networking skills to foster relationships with referral sources
